Medical & Prescription Plan Options

Pick from one of two different medical plan choices. Each is structured differently in terms of copays, premiums and deductibles.

Core PPO Plan

The Core PPO Plan offers traditional coverage for health care services at the lowest cost to employees. This plan has copays for office visits, urgent care, and prescription drugs. The plan has the highest deductible and lowest weekly premiums. Preventive care is covered at 100 percent.

Health Investment Plan

The Health Investment Plan offers a high-deductible and is eligible for a Health Savings Account. All services, including prescriptions, are subject to your deductible. Preventive care is covered at 100 percent.

Pre-Tax Savings Accounts

In 2024, the maximum amount you can contribute to either a Health Savings Account or a Flexible Spending Account will increase. What type of these pre-tax savings accounts you can contribute to is determined by the health care plan you choose.

Optum Financial will continue to administer the FSA and HSA in 2024.

Dental & Orthodontia Plan

The Gordon Food Service Dental Plan is administered by Delta Dental of Michigan. Dental is purchased separately from medical and vision coverage. To locate an in-network dentist, visit www.deltadentalmi.com and click on “Find a Dentist”.

Delta Dental ID Cards Provided but not Required to Access Coverage

When you seek services from an in-network Delta Dental provider, they can verify coverage with the following information: Employee Social Security Number, Plan 1800, (800) 524-0149

Benefits of Using
In-Network Dentists

To maximize the benefits available under the plan, Gordon Food Service has partnered with Delta Dental of Michigan to offer services for a reduced fee if an in-network dentist is used. The dental network consists of Delta Dental PPO and the Delta Dental Premier networks. Dentists outside the network may be used with the same dental benefit coverage; however, you will not receive a reduced rate for those services and may be billed for services over what the plan covers.
